Before any changes take place, risk assessment of the change has to be completed. Risk Assessment is based on the complexity of the change. It could be simple as answering one or two questions or complex enough for an organization to conduct a full process hazard analysis or HAZOP. Second most important item during MOC is to determine if the project is "permanent" or "temporary". It has been known that many "temporary" projects have become "permanent" in nature. A complete MOC process requires reminders to complete Employee Notification, Training, updating Emergency Response & Planning, etc. In short, a MOC can not be closed-out until all the required documentation has been completed.
